FAMILY LIVING SERVICES
Provided by natural and surrogate families who are approved as family living providers under the Medicaid Developmental Disabilities (DD) Waiver program.

Key features of the program:
• 24/7 care for adults 18+ years of age.
• Family provider is responsible for all personal care.
• Family provider receives respite services for the individual.
• Family provider must complete training mandated by the Department of Health.
• Home based services are provided in the home of the approved provider.
