IS MY FAMILY ELIGIBLE FOR SERVICES?
Alta Mira serves individuals who meet at least one of the following definitions. Note: A child may be served under these criteria through age three (3).
(1) DEVELOPMENTAL DISABILITY
A severe chronic disability of a person, which is attributable to a mental or physical impairment or a combination of mental and physical impairments, before the age of 21.
(2) DEVELOPMENTAL DELAY
A significant delay in one or more the following areas: cognitive, speech/language, physical/motor. A marked difference between the chronological age and developmental age.
(3) ESTABLISHED CONDITION
A diagnosed physical, mental, or neurological condition that has a high probability of resulting in a developmental delay. Examples of established conditions include Down Syndrome, autism, cerebral palsy, vision and hearing impairments or fetal alcohol syndrome.
(4) AT-RISK FOR DEVELOPMENTAL DELAY
Early medical condition known to cause possible developmental delays: premature birth; low birth weight; maternal chemical dependence; also physical, social, economic or environmental threats.
After age three (3), the family must contact the Department of Health (DOH) to verify eligibility for continued developmental disability services:
